DAILY HEALTH CHECK FORM
If your child has one symptom that persists for more than 24 hours, a fever, or two or more symptoms from the list, they should be assessed to determine if they should seek testing for COVID-19.
Have you or anyone in your household traveled to any countries outside of Canada, including the United States, within the last 14 days?
Has your child, or anyone in your household, had close contact with someone who has had possible exposure to the COVID-19 virus?
If you answered “yes” to one or both of the questions above but do not currently have any symptoms:
The Ministry of Health and BCCDC state that you need to quarantine (self-isolate) for 14 days and monitor
yourself for new symptoms.
If you answered “yes” to one or both of the questions above and have developed symptoms:
The Ministry of Health and BCCDC state that you need to quarantine (self-isolate) and avoid contact with others for at least 10 days after the onset of symptoms.
(If you choose not to have your child tested) Sweet Cherry Blossom Daycare requires that your child stay at home for at least 14 days. It is very important that the child complete the entire 14-day self-isolation period. This is because the child may still be able to spread COVID-19 during this time, even if the child does not present with symptoms.
